Registered Nurse (RN) - Home Health (Part-Time)

We are seeking an experienced and passionate Registered Nurse (RN) to join our Indiana team in a part-time capacity. What You'll Do As a Home Health Registered Nurse, you will play a vital role in improving patient outcomes through skilled nursing care, education, and care coordination.
Responsibilities include:
Conduct comprehensive patient assessments and ongoing evaluations of symptoms, progress, and care needs. Perform initial home health visits and complete reassessments as needed. Develop, implement, and monitor individualized care plans. Accurately complete and submit OASIS documentation in a timely manner. Collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure coordinated patient care. Supervise and provide guidance to Home Health Aides (HHAs) when appropriate. Monitor patient conditions and promptly communicate significant changes to the physician and care team. Ensure compliance with Medicare regulations, agency policies, and industry standards. Educate patients and caregivers on disease management, medications, treatments, and care plans. Deliver skilled nursing services in the home setting, including wound care, IV therapy management, and other clinical interventions. Maintain accurate, detailed, and timely documentation of all patient care activities. Qualifications:
Graduate of an accredited nursing program; BSN preferred. Active and unrestricted Registered Nurse (RN) license in the state of Indiana. Minimum of one (1) year of acute care, home health, or related clinical experience. Current CPR certification. Valid driver's license, reliable transportation, and ability to meet agency insurance requirements. Strong clinical assessment, organizational, and critical-thinking skills.
