In this vital role, you will visit Medicare Advantage plan members in their homes, delivering comprehensive assessments that support long-term health and wellness. Each evaluation, taking approximately 45 minutes, may serve as an annual review or post-discharge follow-up, empowering you to make a significant difference in the care continuum.

Responsibilities:

Perform annual risk assessments for Medicare Advantage plan members in home settings Thoroughly review medical history, current conditions, and all medications Educate patients on diagnoses and prescribed treatments Identify gaps in care and offer recommendations to resolve them Documentation and reportingwithout treatment plan development or medication prescribing

Qualifications:

Active Nurse Practitioner license in Indiana Board certified by AANP or

ANCC MSN

from an accredited nursing school Current Basic Life Support (BLS) certification At least 6 months of Nurse Practitioner experience preferred
