Job Description

Help at Home is seeking a compassionate Registered Nurse to deliver high-quality home-based care within our Healthcare & Medical Services team. In this full-time role, you will assess client needs, develop and implement individualized care plans, administer medications and treatments, and closely monitor changes in health status. You'll collaborate with families and interdisciplinary teams to promote safety, independence, and dignity. Responsibilities Provide skilled nursing care to clients in their homes, following individualized care plans Conduct comprehensive health assessments and monitor changes in condition Administer medications and treatments safely and accurately Document care in electronic health records in accordance with regulatory standards Collaborate with physicians, therapists, and caregivers to coordinate services Educate clients and families on disease management, medications, and safety Promote client independence while ensuring comfort, dignity, and safety Adhere to infection control, quality, and ethical standards of practice Participate in care conferences and contribute to care plan updates Support Help at Home's culture of integrity, compassion, and innovation Required Skills Patient assessment Care planning and coordination Medication administration Wound care IV therapy and venipuncture Electronic health records (EHR) documentation Infection control procedures Vital signs monitoring Home safety evaluation Patient and family education
