Director of Nursing Position Summary:
We're looking for a dedicated and knowledgeable Registered Nurse (RN) to deliver skilled home health services. In this role, you'll provide compassionate, patient-centered care right in patients' homes while ensuring full compliance with Indiana Medicaid requirements. You'll partner closely with patients and care teams to carry out personalized care plans that promote health, independence, and a better quality of life.
Key Responsibilities:
Provide skilled nursing services, including wound care, medication administration, chronic disease management, and patient education. Ensure all services are medically necessary and properly documented to meet Medicaid reimbursement standards. Coordinate care with physicians, case managers, and other members of the interdisciplinary team. Empower patients and caregivers with knowledge on disease processes, medication regimens, and self-care strategies. Maintain accurate, timely, and compliant documentation within the electronic health record (EHR). Take part in Medicaid audits, quality assurance initiatives, and case conferences. Champion patient needs and help connect them with community resources and support services.
Qualifications:
Current and unrestricted Registered Nurse (RN) license in the state of Indiana. At least 1 year of nursing experience; home health and Medicaid experience strongly preferred. Solid understanding of Indiana Medicaid policies, documentation standards, and prior authorization processes. Current CPR certification. Valid driver's license and reliable transportation. Sharp clinical judgment along with strong communication and organizational skills. Familiarity with electronic documentation systems preferred.
Working Conditions:
Regular travel to patient homes throughout assigned service areas. Exposure to a variety of home environments and potential allergens. Flexible schedule that may include evenings or weekends based on patient needs.
