Full-Time Position Overview The Orthopedic Care Coordinator, RN is responsible for coordinating the patient journey across the orthopedic care experience. This role combines clinical nursing, patient navigation, care coordination, and onsite clinic support. The Orthopedic Care Coordinator will work closely with orthopedic providers, patients, facility teams, and internal HPM teams to ensure patients are prepared for treatment, understand their care plans, receive appropriate follow-up, and do not get lost in the healthcare process. This is an excellent opportunity for an experienced RN who enjoys both direct patient interaction and the behind-the-scenes coordination required to make excellent care happen. Key Responsibilities Patient Navigation & Care Coordination Coordinate the patient journey from initial scheduling through treatment and follow-up, maintaining accountability for timely progression and completion of identified next steps. Help patients understand their care plans, treatment recommendations, next steps, and follow-up requirements. Maintain ownership of an assigned patient population and proactively ensure patients continue progressing through their care plans. Identify and resolve barriers that could delay or disrupt care. Coordinate scheduling, follow-up appointments, referrals, orders, and other patient needs. Clinic & Provider Support Travel to assigned regional facilities on orthopedic clinic days. Provide clinical nursing support to orthopedic providers and assist with efficient patient flow. Reinforce provider treatment plans and ensure patients understand their recommendations. Coordinate orders, follow-up needs, and next steps before patients leave the facility. Build strong working relationships with orthopedic providers, facility staff, and healthcare teams. Pre-Procedure Readiness & Patient Education Coordinate required preparation for procedures and treatment. Coordinate, track, and follow up on required clearances, testing, documentation, and other prerequisites to support timely procedural readiness. Provide education regarding procedures, medications, preparation, and post-procedure expectations. Identify incomplete requirements early and proactively address issues that could result in delays or cancellations. Post-Procedure Follow-Up & Outcomes Conduct appropriate clinical follow-up following procedures. Assess patient recovery and treatment response. Identify patient concerns and determine when provider escalation is appropriate. Coordinate subsequent care based on the patient's treatment plan. Track patient outcomes, treatment response, and completion of required follow-up. Continuity & Patient Experience Monitor patients throughout the care pathway and identify patients who have stalled or fallen out of follow-up. Assist with recovery of cancellations and missed appointments. Help patients overcome barriers to completing recommended treatment. Promote a consistent, patient-centered experience across facilities and providers. Look for opportunities to improve care coordination and patient progression. Qualifications Required Active, unrestricted RN license. BLS certification. Minimum 3 years of RN experience, including experience in orthopedic, surgical, procedural, musculoskeletal, or a closely related clinical setting. Experience coordinating patients across multiple stages of care, including pre-procedure preparation and post-procedure follow-up. Strong clinical assessment, triage, and judgment skills. Strong EMR and clinical documentation skills. Ability to work independently and manage multiple patients and competing priorities. Experience collaborating with providers and interdisciplinary healthcare teams. Valid driver's license and ability to meet organizational requirements for regular travel among assigned facilities. Preferred 5+ years of RN experience. Orthopedic or surgical nursing experience. Experience in orthopedic clinic or surgery settings, pre-admission testing/pre-operative nursing, surgical navigation, procedure coordination, care management, or nurse navigation. Experience working with patient populations across an entire episode of care. They anticipate barriers, close loops, and maintain visibility into what needs to happen next for each patient while using sound clinical judgment regarding when provider escalation is necessary. Work Environment & Travel This is a regional position supporting HPM's orthopedic program at participating facilities. The role will include regular travel based on orthopedic clinic schedules and patient needs.

Locations:

Regional/Facility-Based - supporting orthopedic services in Hoisington, Minneola, Anthony, and Plainville, Kansas, with additional regional locations as business needs evolve. The position will include both onsite clinic days and coordination work between clinic days. On clinic days, you will travel to the assigned facility, support the orthopedic provider and patient flow, provide clinical nursing support and education, coordinate orders and next steps, and ensure patients leave with a clear understanding of their care plan. Between clinic days, you will coordinate upcoming patients, prepare patients for procedures, track clearances and testing, provide education, follow up with patients after procedures, document outcomes, and resolve barriers to continued care. Pay:

$24.00 - $36.00 per hour
