Key Responsibilities:

Perform in-home risk assessments, averaging 45 minutes each, for Medicare Advantage members Review and evaluate prior/current medical history, chronic conditions, and medication regimens Offer patient education regarding health conditions and prescribed medications Identify gaps in patient care, providing recommendations for improvement

this position does not require creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Kansas Board Certification through AANP or ANCC Current BLS (CPR) certification MSN from an accredited nursing program At least six months of recent experience as a Nurse Practitioner is preferred In this role, you will receive a competitive rate of $80$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
