Job Description
Job Summary:
Coordinates the interdisciplinary team in resident assessment and development of resident plan of care according to the Resident Assessment Instrument (RAI) and state and federal requirements.Job Responsibilities:
Assess residents in order to accurately complete the Minimum Data Set and Resident Assessment Protocol.Coordinate MDS and Care Plan TeamImplement and monitor individual care plans for residents based on MDS Guidelines.
Maintain a schedule of due dates for MDS and Care Plans including new admissions and those with significant changes.
Submit the MDS to the appropriate agency.
