Job Description
Ascension HomeHealth Care is seeking a full-time Registered Nurse Case Manager to join our team! As a Case Manager, you will be responsible for the management of a high volume of care cases. You will be responsible for the following duties: Review and analyze patient records to determine diagnosis and treatment plan. Review and evaluate physician orders to ensure that they are in compliance with patient care plan. Review and document communication with referring physicians, as well as other healthcare professionals. Collaborate with other case managers to ensure that all cases are coordinated and coordinated effectively. Provide case management support to patients, their family members, and other stakeholders involved in their care planning. Documenting all interactions, communications, and case management activities in the electronic medical record. Participate in ongoing training and education to maintain knowledge and skills necessary for effective case management. This is a full-time position that requires Monday - Friday availability from 8:00am - 5:00pm.
Experience:
•Registered Nurse (RN) or Bachelor's Degree in Nursing (or equivalent) required.
•2+ years of experience in home health setting required.
Education:
•High School Diploma, or equivalent work experience required.

$70,000.00 - $90,000.00 per year
