Key Responsibilities:

Conduct comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members Review and document prior/current medical histories, conditions, and medication profiles Educate patients and families on chronic and acute medical conditions and prescribed medications Identify care gaps and make actionable recommendations to improve member care No requirement to create formal treatment plans or prescribe medications

Qualifications:
