Key Responsibilities:

Conduct thorough annual home health risk assessments for Medicare Advantage members, with each visit averaging 45 minutes Evaluate past and current medical history, existing conditions, and medication use Deliver clear education to patients on managing their medical conditions and understanding medications Identify care gaps and recommend potential resolutions to improve overall patient outcomes No requirement to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Kansas (KS) Current CPR (BLS) certification National board certification from AANP or ANCC Minimum of 6 months experience as a Nurse Practitioner preferred MSN degree from an accredited nursing school In return for your expertise, enjoy compensation of $80-$90 per evaluation, in addition to a mileage stipend and payment for administrative duties.
