Duties:
```
  • Provide direct patient care and case management services to assigned patients
  • Conduct comprehensive assessments of patients' medical conditions and needs
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Monitor and evaluate patients' progress towards achieving health goals
  • Coordinate and facilitate communication between patients, families, healthcare providers, and community resources
  • Conduct regular patient visits to assess their needs and provide support and education
  • Collaborate with physicians and other healthcare professionals to ensure optimal patient outcomes
  • Utilize medical records and documentation systems to maintain accurate patient information
  • On-call rotation or Full-time on-call position available.

Qualifications:
```
  • Current Registered Nurse (RN) license or LPN license in the state of Kansas
  • Minimum of 1 years of experience in long-term care setting or home health.
  • Strong knowledge of discharge planning, primary care, and patient care coordination
  • Familiarity with utilization management principles and processes
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Proficient in medical record documentation and electronic health record systems
  • Solid understanding of human anatomy, physiology, and medical terminology
