Job Description Position Summary:
The Intake Registered Nurse (RN) conducts initial assessments to determine whether the company can meet a potential client's personal care needs, including ADL and IADL support within the scope of HCBS. The Intake RN serves as the primary liaison among patients, families, care coordinators, and internal departments to support a smooth, timely start of service. This role collaborates with the Intake Manager to ensure new patients are clinically ready, receive continuous care, and meet regulatory requirements.
Supervision:
Works under the supervision of the Director of Nursing, Intake Manager, and Chief of Staff.
Education and Licensure:
  • Associate's Degree in Nursing required; Bachelor of Science in Nursing (BSN) preferred.
  • Active Registered Nurse (RN) license in good standing with the Kansas State Board of Nursing.
Experience:
  • Minimum of two (2) years of RN experience required.
  • Five (5) years of RN experience preferred.
  • Home health or community-based care experience preferred.

  • Professional leadership and clinical decision-making skills.
  • Provide initial patient assessments and evaluations, completing visits and documentation within 48 hours of assignment.
  • Educate patients and families on proper home health care strategies and procedures according to the plan of care and HCBS regulations.
Performance Expectations:
Performance will be evaluated based on:
  • Timeliness and accuracy of clinical documentation.
  • Quality and consistency of patient assessments and supervisory visits.
  • Patient safety and satisfaction outcomes.
  • Effectiveness in supporting timely patient admissions.
  • Compliance with agency policies and state regulations.
  • Professionalism and communication with patients, families, and team members.

Skills and Requirements Intake Support Responsibilities:
  • Collaborate with the Intake Coordinator to support timely and efficient patient admissions.
  • Assist in reviewing referral information for clinical appropriateness and service needs.
  • Support the collection and verification of required clinical documentation, including physician orders as directed.
  • Communicate with patients, families, and referral sources as needed to facilitate the intake process.
  • Provide clinical input to assist in determining patient care needs and service planning.
  • Coordinate with internal staff to support timely scheduling and initiation of services.
  • Ensure all intake-related documentation is accurate, timely, and compliant with agency policies and state regulations.
  • Identify and communicate clinical concerns or barriers to admission to the Intake Coordinator and/or leadership.
