Job Description
Signature HealthCARE seeks an MDS Coordinator to lead the RAI/MDS process in our long-term care and post-acute community. In this resident-centered, mission-driven environment, you'll complete and manage accurate assessments, coordinate interdisciplinary care planning, and support regulatory compliance and reimbursement. You will collaborate closely with nursing, therapy, and support teams, educate staff on MDS and documentation standards, and contribute to quality improvement initiatives that enhance dignity, outcomes, and quality of life for our residents.
