Schedule:

Full-time, Monday through Friday On-call coverage may be required Daily visit expectations providing variety and structure

Required Qualifications:

Current, active RN license At least 1 year of recent home health RN experience Proven OASIS experience (required) Proficiency with HomeCare HomeBase is strongly preferred Reliable transportation and valid drivers license Able to confidently manage a busy, structured schedule

Key Responsibilities:

Deliver skilled in-home nursing care to patients based on individualized plans Complete patient assessments and enter OASIS documentation accurately and promptly Administer wound care, manage medications, educate patients, and monitor disease progress Collaborate with physicians and interdisciplinary care teams, updating patient care plans as needed Communicate clearly about patient status and changes to the clinical team Provide compassionate support and education to patients and families
