Cardinal Hill Rehabilitation Hospital, part of Encompass Health, is seeking a Dialysis RN to provide specialized hemodialysis care within our Healthcare & Medical Services department. In this role, you'll deliver evidence-based dialysis treatments, monitor patient responses, and collaborate with an interdisciplinary rehab team to optimize outcomes. Responsibilities Perform comprehensive pre- and post-dialysis nursing assessments for each assigned patient Initiate, monitor, and terminate hemodialysis treatments according to physician orders and established protocols Manage and monitor vascular access sites, including fistulas, grafts, and central lines, ensuring patency and infection prevention Administer prescribed medications, IV therapies, and blood products as ordered during dialysis sessions Continuously monitor patient responses to treatment, including vital signs, hemodynamic status, and symptoms, intervening promptly when needed Interpret laboratory values related to kidney function, electrolytes, and fluid balance, and communicate significant changes to the provider Develop, implement, and adjust individualized nursing care plans in collaboration with the interdisciplinary team Document all assessments, interventions, patient responses, and education in the electronic medical record accurately and in a timely manner Educate patients and families on dialysis procedures, treatment goals, fluid and dietary restrictions, and home care management Maintain strict infection control and safety standards, including equipment checks, water quality verification, and environmental cleanliness Collaborate with physicians, nephrologists, therapists, dietitians, and case management to coordinate holistic patient care in the rehab setting Participate in interdisciplinary rounds, care conferences, and quality improvement initiatives related to dialysis and patient outcomes Respond to emergencies and acute changes in patient condition, utilizing strong critical thinking and clinical judgment Support a culture of teamwork, respect, and open communication within the dialysis unit and across hospital departments Required Skills Hemodialysis treatment setup, initiation, monitoring, and termination Vascular access management (fistulas, grafts, central venous catheters) Acute and chronic kidney disease management fundamentals Interpreting lab values related to renal function, electrolytes, and fluid balance IV therapy and medication administration during dialysis Electronic medical record (EMR) documentation Infection prevention and control in dialysis settings Patient and family education on dialysis and renal health Assessment and monitoring of hemodynamic status and vital signs Management of dialysis-related complications (hypotension, cramps, access issues, etc.) Interdisciplinary collaboration and care coordination Critical thinking and clinical decision-making in acute situations
