Qualifications:

Responsibilities:

Conduct comprehensive patient assessments in the home Develop, update, and implement individualized care plans Administer medications, wound care, and other nursing interventions as prescribed Monitor, record, and report changes in patient conditions Educate patients and families on disease management and at-home care techniques Coordinate care with physicians, therapists, and other healthcare team members Accurately complete electronic or paper charting and comply with regulatory standards Ensure a safe, clean, and supportive home environment
