Schedule:

Full-time, Monday through Friday On-call probable

Key Responsibilities:

Deliver skilled nursing care, clinical interventions, and patient education directly in patients homes Accurately complete and submit OASIS assessments and documentation on time Collaborate with physicians and interdisciplinary teams to create and implement patient care plans Perform wound care, manage medications, monitor chronic conditions, and assess disease progression Provide clear communication regarding patient condition changes with the clinical team Travel between patient locations, maintaining professionalism and timeliness

Desired Qualifications:

Active RN license Minimum of one year recent home health RN experience OASIS expertise required Strong proficiency with HomeCare HomeBase (preferred) Capable of managing daily patient visits with attention to detail and documentation Reliable transportation and valid drivers license
