Job Description
Signature HealthCARE is seeking a compassionate Registered Nurse to join our Healthcare & Medical Services team. In this role, you will deliver person-centered nursing care, coordinate with an interdisciplinary team, and support residents in long-term care and post-acute rehabilitation settings. Responsibilities include assessments, care planning, medication administration, documentation, and family communication. Responsibilities Perform comprehensive nursing assessments and ongoing monitoring of residents' and patients' clinical status Administer medications, treatments, and therapies in accordance with physician orders and nursing standards of practice Develop, implement, and regularly update individualized care plans in collaboration with the interdisciplinary team Document all assessments, interventions, and outcomes accurately and timely in the Electronic Health Record (EHR) system Monitor for and respond to changes in condition, escalating concerns to providers and leadership as appropriate Educate residents, patients, and families about diagnoses, medications, and care plans, promoting understanding and engagement in care Coordinate care with physicians, nurse practitioners, therapists, social workers, and nursing assistants to ensure seamless, resident-centered services Support and mentor nursing assistants and new team members, promoting a culture of collaboration, respect, and continuous improvement Adhere to infection prevention, safety, and regulatory standards, participating in audits and quality improvement initiatives Advocate for resident rights, dignity, and comfort, contributing to a positive, compassionate care environment Required Skills Patient assessment Medication administration Care planning Wound care IV therapy Electronic Health Records (EHR) documentation Patient and family education Clinical triage Infection prevention and control Vital signs monitoring Interdisciplinary care coordination Discharge planning Time management Clinical decision-making Basic life support
