Schedule:

Full-time, Monday through Friday On-call likely as part of the rotation Structured daily visit requirements

What Youll Do:

Deliver high-quality in-home nursing care and skilled interventions Complete comprehensive patient assessments and submit OASIS documentation promptly and accurately Collaborate with physicians and interdisciplinary care teams to develop and implement individualized care plans Provide wound care, medication administration, disease management, and patient/family education Keep the clinical team informed of any changes in patient condition

Qualifications:

Active RN license Minimum of 1 year of recent home health RN experience OASIS documentation expertise (required) Proficiency with HomeCare HomeBase (strongly preferred) Strong organizational skills to meet full-time daily visit requirements Reliable transportation and a valid drivers license
