Schedule:

Full-time, Monday through Friday On-call duties as needed

What Youll Do:

Deliver direct, in-home nursing care and skilled interventions Complete thorough patient assessments and OASIS documentation accurately and promptly Coordinate with physicians and multidisciplinary care teams to create individualized care plans Provide wound care, medication administration, patient and caregiver education, and monitor chronic diseases Ensure timely communication of changes in patient conditions to the care team

Preferred Qualifications:

Active Registered Nurse (RN) license At least one year of recent experience as an RN in home health Demonstrated OASIS proficiency Experience with HomeCare HomeBase software (strongly preferred) Able to manage a consistent, structured patient schedule Reliable transportation and a valid drivers license
