A skilled and compassionate Registered Nurse with home health experience is needed for a field-based contract role serving patients in their homes within the assigned territory. This full-time, 13-week position focuses on promoting patient-centered care, clinical excellence, and continuity as part of an interdisciplinary team.

Monday Friday, 8:00 AM 5:00

PM Key Responsibilities:

Conduct comprehensive OASIS assessments and develop individualized care plans. Deliver skilled nursing interventions including wound care, medication administration, IV therapy, and management of chronic conditions. Monitor and document patient progress in Home Care Home Base (HCHB). Update the care team and physicians promptly regarding status changes. Educate patients and caregivers about diagnoses, medications, and preventive measures. Collaborate closely with therapists, social workers, and aides to coordinate services. Attend case conferences and team meetings as needed. Maintain compliance with all documentation, regulatory, and care delivery standards.

Qualifications:

Active and unrestricted RN license in the practicing state. Minimum of 1 year recent field-based home health RN experience. Proficient with Home Care Home Base (HCHB) and OASIS assessments. Strong assessment, clinical judgment, and organizational skills. Reliable transportation with valid drivers license and auto insurance. Ability to work independently, communicate effectively, and efficiently manage a flexible field schedule. BSN and experience with Medicare standards, highly preferred.
