Key Responsibilities:

Visit members at home to conduct thorough annual health risk assessments Review members previous and current medical history, including medications and related conditions Educate and inform members about their health conditions and existing medication regimens Identify and report any gaps in care, providing recommendations for next steps No requirement to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner License in Kentucky Current CPR (BLS) certification Board Certification (AANP or ANCC) Six months or more of recent nurse practitioner experience preferred MSN from an accredited nursing school
