Key Responsibilities:

Deliver in-home nursing care, skilled interventions, and patient education Complete timely, accurate patient assessments and OASIS documentation Collaborate with physicians and multidisciplinary teams to implement tailored care plans Perform wound care, medication management, and continuous disease monitoring Maintain clear communication with patients, families, and clinical colleagues regarding condition changes and updates

Qualifications:

Active Registered Nurse (RN) license At least one year of recent RN experience in home health settings Demonstrated proficiency with OASIS documentation Experience with HomeCare HomeBase (strongly preferred) Reliable transportation and a valid drivers license Ability to manage a structured, full-time caseload and travel between home environments
