PM Key Responsibilities:

Conduct comprehensive patient assessments using OASIS, develop individualized plans of care, and perform skilled nursing interventions, including wound care, IV therapy, and chronic disease management. Monitor patient progress, document all care in Home Care Home Base (HCHB), and proactively communicate changes with medical providers. Educate patients and families about conditions, treatments, and disease management strategies to empower self-care. Coordinate with physical therapists, occupational therapists, social workers, and aides, ensuring well-integrated care. Participate in case conferences, team meetings, and ongoing professional development. Adhere to clinical, regulatory, and documentation standards while managing independent scheduling and daily travel within your assigned territory.

Qualifications:

Active RN license (state of practice) At least 1 year recent experience in home health nursing; field experience preferred Demonstrated proficiency in HCHB and OASIS Strong assessment, communication, organizational, and problem-solving abilities Valid drivers license, reliable transportation, and auto insurance BSN preferred; knowledge of Medicare and home health compliance is a plus Benefits include weekly pay, multiple insurance options, 401(k) participation, 24/7 support, and mileage reimbursement.
