A leading healthcare provider is seeking a dedicated Registered Nurse (RN) with expertise in home health and experience with OASIS for a 13-week full-time contract. Schedule and Hours:

Full-time contract (13 weeks), with potential extension Monday through Friday schedule; on-call duties may also be assigned Daily visit targets and structured expectations

Key Responsibilities:

Deliver in-home patient care including skilled nursing interventions Complete accurate, timely OASIS documentation and patient assessments Develop and implement individualized care plans in partnership with physicians and interdisciplinary teams Carry out wound care, medication administration, disease process monitoring, and patient/family education Maintain frequent communication with the clinical team to report changes in patient conditions

Desired Qualifications:

Active and unrestricted RN license Minimum 1 year of recent home health RN experience Proficient in OASIS documentation (required) Experience with HomeCare HomeBase software (strongly preferred) Reliable personal transportation and valid drivers license Ability to independently manage a full-time schedule and daily patient visits
