Schedule:

13-week contract, with potential to extend Full-time hours: Monday through Friday On-call duties may be required

Key Responsibilities:

Deliver in-home nursing care, including patient assessments and skilled interventions Accurately complete OASIS documentation and submit within required timelines Collaborate with physicians and multidisciplinary care teams to create and implement patient-specific plans Manage wound care, medication administration, disease monitoring, and provide thorough patient/family education Ensure timely communication of any changes in patient status to the clinical team

Who You Are:

Active, unencumbered RN license Minimum one year of recent home health RN experience Demonstrated OASIS documentation proficiency (required) Experience with HomeCare HomeBase (highly preferred) Reliable transportation and valid drivers license Ability to work a structured, full-time schedule and efficiently manage daily visit expectations
