13-week full-time contract, Monday through Friday Possible on-call requirements Daily patient visit expectations

Key Responsibilities:

Deliver in-home nursing care and skilled interventions to homebound patients Conduct timely, thorough assessments and complete OASIS documentation Collaborate closely with physicians and interdisciplinary care teams to implement individualized care plans Manage wound care, medication administration, patient education, and disease process monitoring Accurately communicate patient updates and changes in condition with the clinical team

Qualifications:

Current, active RN license At least 1 year of recent home health RN experience Strong OASIS documentation skills (required) Proficiency with HomeCare HomeBase (highly preferred) Reliable transportation and a valid drivers license Skilled at organizing a full-time schedule and managing daily patient visits
