This is an on-site, full-time position with daily travel to patient homes. with a current resume for consideration.

Pay:

$54 to $56

Type:

Three-month Contract with possibility of an extension

Schedule:

Monday

• Friday (8:00 AM
• 4:30 PM)

Coverage Areas:

Shelbyville, KY including Shelby County and parts of Eastern Jefferson County

Primary Responsibilities:

• Provide skilled nursing services in the home health setting, including start of care admissions,prehensive assessments, and ongoing patient management in accordance with physician orders
• Conduct patient assessments and develop individualized care plans based on clinical findings and patient needs
• Ensure accurate, timely, andpliant documentation of all patient care activities in Homecare Homebase (HCHB) EMR system
• Coordinate care with physicians and interdisciplinary healthcare team members to ensure continuity and quality of care
• Monitor patient conditions, identify changes in status, and implement appropriate nursing interventions
• Educate patients and caregivers on disease management, medications, and home care instructions

Qualifications:

• Active Registered Nurse (RN) license required
• Minimum 1 year of home health experience required
• Experience with Homecare Homebase (HCHB) EMR required
• Current CPR certification required
• Valid driver's license, current auto insurance, and access to a reliable vehicle required Why work for Adecco?

Pay Details:
