We are seeking a dedicated Registered Nurse with significant home health and OASIS experience for a 13-week, full-time contract. Schedule:

13-week assignment with potential to extend Full-time, Monday through Friday On-call responsibilities may be included Structured daily visit expectations

Key Responsibilities:

Deliver in-home nursing care and skilled clinical interventions Conduct patient assessments and complete timely, accurate OASIS documentation Collaborate with physicians and interdisciplinary care teams to create individualized care plans Provide wound care, manage medications, offer disease education, and monitor patient progress Clearly communicate changes in patient conditions to the clinical team

Qualifications:

Active RN license At least one year of recent experience in home health RN roles Proficiency with OASIS documentation is required Experience with HomeCare HomeBase software is strongly preferred Able to maintain a structured, full-time schedule with multiple patient visits each day Reliable transportation and a valid drivers license
