In this role, you will deliver person-centered nursing care, coordinate with an interdisciplinary team, and support residents in long-term care and post-acute rehabilitation settings. Responsibilities Provide direct, resident-centered nursing care aligned with individualized care plans. Perform comprehensive and ongoing assessments, including vital signs, condition monitoring, and risk evaluations. Administer medications, treatments, and therapies safely and accurately in accordance with physician orders and facility protocols. Document assessments, interventions, and resident responses in electronic health records (EHR) in a timely and accurate manner. Collaborate with physicians, therapists, social workers, and other team members to coordinate and optimize resident care. Educate residents and families on diagnoses, medications, treatment options, and self-care strategies. Monitor changes in resident condition, communicate concerns promptly, and initiate appropriate interventions. Support discharge planning and transitions of care to ensure continuity and safety. Adhere to infection control, safety, and regulatory standards, including state and federal long-term care requirements. Provide guidance and support to nursing assistants and other support staff to ensure consistent, high-quality care. Participate in quality improvement and innovation initiatives aimed at enhancing resident outcomes and experiences.
