Role Overview:

Conduct in-home risk assessments for Medicare Advantage members, including annual health evaluations and post-discharge follow-ups Review and evaluate prior and current medical history, chronic conditions, and medication use Provide patient education about diagnoses, preventive care, and medication management Identify gaps in care and offer clinical recommendations without developing treatment plans or prescribing medications Visits are scheduled throughout Bogalusa, Franklinton, Angie, and surrounding areas within

Washington Parish Key Qualifications:

Unencumbered Nurse Practitioner license in Louisiana Active Board Certification (AANP or ANCC) Valid BLS (CPR) certification Minimum of six months experience as a Nurse Practitioner preferred MSN degree from an accredited nursing program
