Make a difference in patient care by performing in-home health risk assessments for Medicare Advantage plan members throughout Washington Parish, Louisiana, including Bogalusa, Franklinton, and Angie. As a Nurse Practitioner in this per diem, field-based role, youll conduct comprehensive assessments that support patients ongoing health and guide further carewithout the pressure of prescribing medications or developing treatment plans.

Key Responsibilities:

Perform annual health assessments or post-discharge visits in patient homes Review medical and medication histories and evaluate prior/current conditions Identify care gaps and provide patient education and recommendations Complete thorough documentation for each 45-minute assessment Travel within Washington Parish to provide assessments in various community locations

Qualifications:

Valid Nurse Practitioner license in Louisiana Current BLS/CPR certification Board Certification via AANP or ANCC At least 6 months experience as a Nurse Practitioner preferred MSN from an accredited nursing program Enjoy earning $80-$90 per evaluation, with an additional mileage stipend and pay for administrative time.
