Job Description

MDS Coordinator (RN or LPN) Deerfield Nursing & Rehabilitation Center - Delhi, Louisiana Join a team dedicated to providing exceptional care to our residents! Deerfield Nursing & Rehabilitation Center is seeking an experienced, detail-oriented MDS Coordinator to join our leadership team. The ideal candidate will have a thorough understanding of the MDS process, Medicare and Medicaid regulations, reimbursement, and interdisciplinary care planning.
Responsibilities:
Coordinate and complete accurate and timely MDS assessments in accordance with federal and state regulations. Ensure compliance with CMS guidelines and RAI Manual requirements. Manage Medicare and PPS assessments while maximizing appropriate reimbursement. Collaborate with nursing, therapy, dietary, social services, and other departments to develop comprehensive care plans. Monitor assessment schedules and ensure all required documentation is completed accurately. Participate in Quality Assurance and Performance Improvement (QAPI) activities. Assist with survey readiness and regulatory compliance. Educate and support interdisciplinary team members regarding MDS documentation requirements. Review medical records for accuracy and completeness.
Qualifications:
Current Louisiana RN or LPN license in good standing. Previous MDS Coordinator experience preferred. Strong knowledge of CMS regulations, Medicare, Medicaid, and PDPM. Excellent organizational, communication, and critical thinking skills. Ability to work independently while collaborating with the interdisciplinary team. Proficient in electronic medical record systems.
