Job Description

Overview We are seeking a dedicated and compassionate Nurse to join our healthcare team. The ideal candidate will possess a strong background in MDS Department and demonstrate expertise in various nursing practices. This role involves providing completion of MDS assessments, care planning among task. The Nurse will be responsible for assessing patient needs, implementing care plans, and collaborating with healthcare professionals to ensure optimal patient outcomes. Duties Conduct comprehensive patient assessments and develop individualized care plans. Administer medications and treatments as prescribed by physicians. Monitor and document patients' vital signs and medical history. Collaborate with interdisciplinary teams to coordinate patient care. Provide education and support to patients and their families regarding health management. Ensure compliance with HIPAA regulations and maintain accurate medical documentation. Participate in discharge planning and follow-up care coordination. Utilize EMR/EHR systems for documentation and patient management. Engage in utilization review and clinical documentation improvement initiatives. Skills Proficient in Point Click Care or similar EMR/EHR systems. Strong understanding of medical terminology, anatomy, physiology, and CPT/ICD coding (ICD-9/ICD-10). Experience in Long Term Care and with insurance authorization process Knowledge of Medicare guidelines, DRG classifications, health standards, and medical management practices. Familiarity with case management processes and discharge planning protocols. Excellent communication skills for effective patient interaction and interdisciplinary collaboration. Ability to perform documentation review while adhering to HIPAA standards. Experience in outpatient services or nursing home environments is a plus.
