Key Responsibilities:

Perform thorough home health risk assessments for Medicare Advantage members Review and evaluate prior/current medical history, medical conditions, and medication use Educate patients regarding their medical conditions and prescribed medications Identify potential care gaps, offering informed recommendations for follow-up or resolution No treatment plan creation or prescribing is required

Required Qualifications:

Active Nurse Practitioner License for Louisiana Current CPR certification (BLS) Board Certification (AANP or ANCC) MSN from an accredited nursing program At least 6 months of recent Nurse Practitioner experience preferred You will receive $80-$90 per evaluation, along with a mileage stipend and compensation for administrative time.
