Schedule:

13-week contract with the potential for extension Full-time, Monday through Friday On-call hours likely as part of the rotation Structured daily visit expectations

Key Responsibilities:

Provide comprehensive in-home nursing care and skilled clinical interventions Complete thorough patient assessments, ensuring precise and timely OASIS documentation Collaborate closely with physicians and multidisciplinary care teams Deliver wound care, medication management, patient and family education, and disease monitoring Communicate promptly regarding changes in patient condition to appropriate members of the clinical team

Qualifications:

Active and unrestricted RN license Minimum of one year recent experience as a home health RN Proven experience with OASIS documentation Proficiency with HomeCare HomeBase is strongly preferred Reliable transportation and a valid drivers license are required Confident managing a structured full-time schedule and daily patient caseload
