Utilize EMR/EHR systems like Epic or Cerner to document all patient interactions accurately and promptly. Coordinate care plans, including discharge planning and case management, to ensure seamless transitions across care settings. Perform specialized procedures such as suturing, ostomy care, or gastrostomy feeding when necessary. Provide triage services by evaluating patient symptoms over the phone or in person to determine urgency and appropriate next steps. Support patients with complex needs including those in memory care, behavioral health, or with developmental disabilities. Maintain compliance with HIPAA regulations and infection control protocols to safeguard patient information and safety. Experience Proven experience working in home care or community health environments with a strong foundation in patient assessment and medical documentation. Critical care experience in settings such as ICU or emergency medicine is highly valued for managing acute situations confidently. Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks is preferred for efficient charting and documentation. Experience working with diverse populations including geriatrics, pediatrics, neonatal ventilator management, or individuals with disabilities enhances your ability to deliver personalized care. Knowledge of advanced nursing procedures such as telemetry monitoring, ventilator management, dialysis support, or infectious disease care is advantageous. Prior exposure to long-term care facilities like nursing homes or assisted living communities demonstrates your ability to handle complex chronic conditions.
