Weeks Schedule:
Full-time, Monday-Friday, 8:00 AM
- 5:00 PM, 40 hours per week (30 units) 13 weeks Mon
- Fri, 8am
- 5pm, 40 hours per week On-call info: On-call required if requested
- on-call needs are subject to change based on staff and branch needs

Responsibilities:
Provide skilled nursing care to patients in their homes, ensuring high-quality and compassionate healthcare. Conduct comprehensive assessments, including OASIS, to evaluate patient needs and track progress. Develop, implement, and update individualized care plans in collaboration with physicians and other healthcare professionals. Utilize Homecare Homebase (HCHB) software to document patient care accurately and efficiently. Educate patients and families on managing illnesses, treatments, and preventive healthcare practices. Maintain compliance with all healthcare regulations, documentation standards, and agency policies. Travel to patient homes, ensuring timely and reliable care delivery.
Qualifications:
Required:
Current RN license or compact license in the state of LA 1-2 years of RN experience in a clinical care setting 2+ years of Home Health experience Homecare Homebase (HCHB) experience OASIS experience Proof of current CPR certification Current driver's license, reliable transportation, and insurance
Preferred:
Bachelor of Science in Nursing (BSN) CHPN certification
Education:
Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma program
