Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage members Review and evaluate patients prior/current medical and medication history Educate patients about their medical conditions and prescribed medications Identify gaps in care and offer recommendations to optimize health outcomes No requirements for treatment planning or prescribing medicationsfocus is on assessment and education

Qualifications:

Active Nurse Practitioner license in Louisiana Board Certification through AANP or ANCC Current BLS (CPR) certification MSN degree from an accredited school of nursing At least 6 months of recent Nurse Practitioner experience preferred This per diem opportunity allows you to maintain a flexible schedule while earning $80-$90 per evaluation, plus a mileage stipend and compensation for administrative time.
