Key Responsibilities:

Perform comprehensive annual or post-discharge home health risk assessments for Medicare Advantage members Review prior and current medical history, ongoing conditions, and medication history Educate patients about their health conditions and medications, encouraging proactive well-being Identify care gaps and provide recommendations for follow-up, empowering better patient outcomes Focus solely on assessmentsno required creation of treatment plans or medication prescribing

Qualifications:

Active Nurse Practitioner license in Louisiana Current Basic Life Support (BLS/CPR) certification Board Certification through AANP or

ANCC MSN
