Key Responsibilities:

Conduct comprehensive annual or post-discharge home health risk assessments Evaluate member medical history, current conditions, and medication usage Provide client-centered education about diagnoses and medications Identify gaps in care and recommend solutions No requirement for creating treatment plans or prescribing medications

Qualifications:

Current Nurse Practitioner License in Louisiana Active CPR (BLS) certification Board Certification via AANP or ANCC At least 6 months experience as a Nurse Practitioner preferred MSN from an accredited nursing program Enjoy competitive compensation of $80$90 per evaluation, with additional stipends for mileage and administrative time.
