What You Will Do:

Coordinate and provide exceptional patient care across all stages—assessment, planning, implementation, and evaluation—in collaboration with the patient, family, physician, and support services. Conduct initial and ongoing physical and psychosocial assessmentsIdentify nursing diagnoses and prioritize patient outcomesDevelop and revise individualized plans of care with interdisciplinary inputDeliver direct patient care and education aligned with plan of careAdminister and monitor treatments, medications, and procedural sedationMaintain thorough documentation in the medical recordEvaluate patient progress and update care plans accordinglyPerform triage assessments and focused assessments across all age groupsEnsure patient safety and compliance with clinical standardsCollaborate on discharge planning and coordinate with support servicesPerform additional duties as assigned
