Schedule:

13-week contract, with possibility for extension Full-time hours, Monday through Friday On-call duties may be required

Key Responsibilities:

Deliver in-home nursing care and skilled interventions for patients in a home health setting Complete comprehensive patient assessments and accurate OASIS documentation on a timely basis Collaborate with physicians and interdisciplinary care teams to develop and implement individualized care plans Provide wound care, patient and family education, medication management, and monitor patient conditions Communicate effectively with the clinical team regarding any changes in patient status

Qualifications:

Active RN license (unrestricted) At least 1 year of recent experience as a home health

RN OASIS

documentation experience is required Proficiency with HomeCare HomeBase software is strongly preferred Dependable transportation and valid drivers license
