Key Responsibilities:

Deliver skilled nursing care, interventions, and patient education in home settings Complete patient assessments and OASIS documentation accurately and promptly Collaborate with physicians and interdisciplinary teams to customize care plans Conduct wound care, medication management, and monitor patient health status Communicate significant developments to clinical team members

Required Qualifications:

Active RN license Minimum 1 year of recent home health RN experience Proficiency with OASIS documentation is essential Experience with HomeCare HomeBase strongly preferred Ability to consistently manage a structured, full-time visit schedule Valid drivers license with reliable transportation
