Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members in Washington Parish Evaluate patients prior and current medical history, ongoing conditions, and medication use Provide clear education on health conditions and medications as needed Identify and report care gaps, recommending actionable solutions

Qualifications:
