Schedule:

13-week contract with the possibility of extension Full-time hours, Monday through Friday On-call participation may be required Structured patient visit volume each day

Key Responsibilities:

Deliver in-home nursing care, including patient assessments and skilled interventions Accurately complete OASIS documentation in a timely manner Partner with physicians and interdisciplinary care teams to design and deliver individualized patient care plans Perform wound care, medication management, disease surveillance, and patient education Clearly communicate all updates and clinical changes to the wider care team to ensure the highest standard of care

Desired Qualifications:

Current, active RN license At least one year of recent home health nursing experience Proven OASIS documentation proficiency Experience with HomeCare HomeBase (strongly preferred) Valid drivers license and reliable vehicle for daily travel Ability to manage a steady, structured caseload with consistent patient visits
