Key Responsibilities:

Deliver in-home nursing care and skilled clinical interventions Conduct thorough patient assessments, ensuring accurate and timely OASIS documentation Collaborate with physicians and the interdisciplinary care team to develop and implement customized care plans Perform essential tasks including wound care, medication administration, patient education, and chronic disease monitoring Maintain effective communication with the care team regarding changes in patient condition

What Were Looking For:

Active RN license Minimum of one year recent experience as a home health RN Proficiency with OASIS documentation is required Familiarity with HomeCare HomeBase (strongly preferred) Ability to independently manage a full-time, structured caseload Reliable transportation and valid drivers license
