Weekday and Weekend Rotation Position Overview We are seeking an experienced Home Health Registered Nurse (RN) to provide compassionate, high-quality patient care throughout the West Monroe service area. This role is responsible for managing patient care in the home setting, maintaining timely and accurate documentation, collaborating with physicians and care teams, and ensuring patients receive exceptional clinical support. The ideal candidate will have prior home health experience, proficiency with Homecare Homebase (HCHB), and the ability to thrive in a fast-paced, field-based environment. Key Responsibilities Provide skilled nursing care and ongoing patient evaluation in the home setting. Communicate effectively with physicians' offices regarding patient conditions, concerns, and changes in care needs. Coordinate patient care with agency staff and interdisciplinary team members. Complete documentation accurately and within required timeframes using the Homecare Homebase (HCHB) system. Assess patient health status and implement appropriate nursing interventions. Educate patients and caregivers regarding treatment plans, medications, and disease management. Participate in weekly case conference meetings to discuss patient progress and care planning. Maintain compliance with agency policies and regulatory requirements. Participate in weekday and weekend on-call rotations.

Required on weekdays and weekends Required Qualifications Active Registered Nurse (RN) license. Previous Home Health nursing experience. Experience utilizing Homecare Homebase (HCHB) documentation system. Strong assessment, clinical, and documentation skills. Ability to work independently while managing a caseload efficiently. Preferred Qualifications Flexible and adaptable in a changing healthcare environment. Strong teamwork and collaboration skills. Excellent communication and interpersonal abilities.
