ACCS Community Mental Health Team Location:
Hyannis, Massachusetts Join our Adult Community Clinical Services (ACCS) team and make a meaningful impact supporting adults with mental health needs as they live independently in the community. As an ACCS Registered Nurse, you'll provide health assessments, coordinate medical care, educate staff and individuals served, and promote overall wellness through a person-centered, recovery-oriented approach. Responsibilities Conduct annual nursing and health assessments and monitor ongoing medical needs. Coordinate care with primary care providers, specialists, hospitals, and behavioral health teams. Educate individuals served, families, and staff on health conditions, medications, and wellness strategies. Support medication management and ensure MAP compliance. Promote healthy lifestyles, chronic disease management, and preventative care. Participate in interdisciplinary treatment planning and crisis support. Maintain accurate clinical documentation and provide on-call coverage as scheduled. Qualifications Current Massachusetts Registered Nurse (RN) license required. Bachelor's degree preferred. Experience in community health, behavioral health, or psychiatric nursing preferred. Valid driver's license 6+ months, reliable vehicle, and willingness to travel within the community. CPR certification required within two weeks of hire.
