Essential Duties and Responsibilities:

Develop a schedule for timely completion of the MDS process according to federal and state regulations. Schedule, coordinate and lead the interdisciplinary care plan meetings. Complete the nursing sections of the MDS assessment and nursing RAPs when applicable. Assure accurate and timely completion of the MDS assessments according to federal and state regulations. Oversee timely signing of MDS sections completed by all interdisciplinary members. Review all MDS assessments and certify completion by signing appropriate MDS sections. Timely transmission of MDS assessments to the state automation database in accordance with state and federal regulations. Receive and review the submission validation report acing upon any corrections needed or assessments rejected. Review the state monthly reports of assessments submitted and act upon any corrections needed. Generate monthly QI (Quality Indicator) reports and furnish copies to the Administrator and Director of Nurses upon request. Participate in all facility surveys conducted by authorized government agencies. Attends state automation MDS update in-services. Develop and implement for general orientation a brief overview of the MDS (Minimum Data Set) process. Committed to working with residents and families to develop and implement strategies that promote person-centered care. Applies "5 Star" customer service approach in all aspects of job. Adheres to all company policies, procedures, and standards of conduct. Reports to the appropriate managers any known or suspected violations of policy and procedures, regulations or standards of conduct. Attends required in-services and training. Performs additional duties as assigned.

Qualifications:

Must be a MA Registered Nurse. Previous experience with the MDS process required. Certification as a Nurse Assessment Coordinator preferred. Knowledge of nursing and medical practice, and the laws, regulations and guidelines covering long-term care. Good organizational skills with the ability to work under pressure and meet deadlines. Must be able to communicate effectively with interdisciplinary team. Must be able to communicate with medical staff, nursing personnel, department directors, residents and families. Must be able to function independently with personal integrity.
